Grandparents have consistently been integral to the survival of families and communities, regardless of national or cultural distinctions, and have also been vital in the preservation of their cultures. In a study of Maori grandparenting in New Zealand, the exploration of meaning and roles of grandparental figures were central to understanding the potential for expanding the discourse on the significance of grandparents across cultures. Seventeen Maori great-great-grandparents, along with their grandparents, who lived in intergenerational households in Aotearoa New Zealand, took part in the interviews. The data analysis leveraged the principles of phenomenology. Five key themes relating to grandparenting emerged from interviews with Maori grandparents, Elders. These themes elucidated the critical role of Elders, encompassing: cultural responsibilities; support systems, resources, and assets; the complexities of sociopolitical and economic circumstances; the current status of their role within families; and the tangible and intangible rewards. Implications and recommendations are offered to advance a more systemic and culturally responsive approach toward supporting grandparents.
Geriatric care in the South-East Asian region, marked by a rapidly growing aging population, necessitates standardized dementia screening tools. The Rowland Universal Dementia Assessment Scale (RUDAS), though employed in Indonesia, lacks demonstrable cross-cultural portability. The Rowland Universal Dementia Assessment Scale (RUDAS) scores were evaluated for reliability and validity within the Indonesian context in this study. One hundred thirty-five Indonesian older adults (52 male, 83 female; age range 60-82) at a geriatric nursing center completed the Indonesian translation of the RUDAS, (RUDAS-Ina), following a content adaptation study involving 35 community-dwelling older adults, nine neurologists, and two geriatric nurses. For the assessment of face and content validity, a consensus-building method was implemented. Subsequent to conducting confirmatory factor analysis, a model with a single factor was revealed by the results. The RUDAS-Ina's score reliability, while marginally acceptable, was deemed sufficient for research purposes (Cronbach's alpha = 0.61). Multi-level linear regression analysis, looking at the association between RUDAS-Ina scores, age, and gender, determined that age was inversely related to RUDAS-Ina scores, meaning lower scores were found in older participants. In a different vein, the variable showed no significant association with gender. Despite the promising results of immune checkpoint inhibitors (ICIs) in late-stage gastric cancer, their application in a neoadjuvant approach lacks large-scale investigation. We evaluated the efficacy and safety profile of neoadjuvant ICI-based regimens in the context of locally advanced gastric cancer.
We investigated patients with locally advanced gastric or gastroesophageal cancer, specifically those treated with ICI-based neoadjuvant regimens. Our investigation encompassed PubMed, Embase, the Cochrane Library, and the proceedings of key international oncology conferences. Our meta-analysis was executed using the META package in R version 36.1.
The search yielded 21 prospective phase I/II studies, which included a total of 687 patients. The rates for pathological complete response (pCR), major pathological response (MPR), and R0 resection were as follows: pCR rate 0.21 (95% confidence interval 0.18-0.24), MPR rate 0.41 (95% confidence interval 0.31-0.52), and R0 resection rate 0.94 (95% confidence interval 0.92-0.96). In terms of efficacy, the highest results were achieved by combining ICI with radiochemotherapy, the lowest with ICI alone, and ICI along with chemotherapy and anti-angiogenesis treatment displayed intermediate efficacy. Patients displaying dMMR/MSI-H characteristics and high PD-L1 levels experienced more pronounced benefits than those demonstrating pMMR/MSS and low PD-L1 expression. The 95% confidence interval for grade 3 or higher toxicity was 0.13 to 0.38, with a point estimate of 0.23. Data from 21 studies (4800 patients) suggest results exceeding those observed in neoadjuvant chemotherapy trials. The pCR rate was 0.008 (95% CI 0.006-0.011), MPR 0.022 (95% CI 0.019-0.026), R0 section 0.084 (95% CI 0.080-0.087), and grade 3+ toxicity 0.028 (95% CI 0.013-0.047).

The management of 20mm non-functioning pancreatic neuroendocrine tumors (PanNETs) is a topic of intense debate and lack of consensus. The varying biological characteristics of these tumors complicate the decision-making process regarding resection versus observation.
A retrospective, multicenter cohort study of 78 patients who underwent resection of 20mm or smaller non-functioning pancreatic neuroendocrine tumors (PanNETs) at three tertiary care centers between 2004 and 2020 investigated the predictive value of preoperative radiological imaging and serological markers in determining the optimal surgical approach for these tumors. Computed tomography (CT) scans revealed a non-hyper-attenuating pattern (heterogeneous/hypodense) on contrast enhancement, along with evidence of main pancreatic duct (MPD) involvement. Elevated serum elastase 1 and plasma chromogranin A (CgA) levels were also detected in blood tests.
In a cohort of small, non-operational PanNETs, lymph node metastasis was observed in 5 of 78 cases (6%), 11 of 76 (14%) were WHO grade II, and 9 of 66 (14%) exhibited microvascular invasion. Critically, 20 out of 78 (26%) displayed at least one of these high-risk pathological elements. The preoperative assessment showed hetero/hypo-attenuation in 25 patients, representing 36% of the 69 assessed, and MPD involvement in 8 patients (11%) of the 76 examined. Elevated serum elastase 1 levels were present in 1 patient (3%) out of a total of 33, and plasma CgA levels were not elevated in any of the 11 patients (0%). The multivariate logistic regression analysis demonstrated a strong correlation between hetero/hypo-attenuation and high-risk pathological factors, with an odds ratio of 61 (95% confidence interval 17-222). In the same multivariate analysis, MPD involvement showed a substantial association with high-risk pathological factors, having an odds ratio of 168 (95% confidence interval 16-1743). Non-functioning PanNETs with detrimental pathological factors were accurately anticipated by the combination of two worrisome radiological hallmarks, with roughly 75% sensitivity, 79% specificity, and 78% accuracy.
Accurately predicting non-functional pancreatic neuroendocrine tumors, which may demand surgical excision, can be achieved via this combination of worrisome radiological indicators.

VP1, VP2, and VP3 are the three viral proteins that make up the non-enveloped canine parvovirus (CPV). This specificity arises from the ability of these VLPs to target transferrin receptors (TFRs) on cancer cells.
